We apologize for any inconvenience this may cause you. 05/11/2005: Originally posted 05/05/2005- As some of you already know, there was a batch of FAA 8500-8 forms distributed last year that were flawed. (See the Notice originally posted in August 2004 located in our Notice Archives for more information) The Education Division has now received a supply of forms the manufacturer has corrected. Some of the flawed forms are still unused. The form numbers of the remaining flawed forms are FF2864000 - FF4363999. Due to ongoing negotions with the manufacturer of these forms, we ask that if you are an AME who received a set of flawed forms and still have a supply that is unused (form numbers FF2864000 - FF4363999), please destroy them. If you have any questions or you need to order a new supply of FAA Form 8500-8, you may contact Gail Gentry in the Education Division at (405) 954-4831. 04/19/2005: ! ! ! SYSTEM ALERT ! ! ! The shadow print (watermark that says "FAA Original Copy", "AME Work Copy", and "Applicant Copy") blocks some of the print on the form. New forms are on their way, and will be recieved shortly. When the new forms are received, the flawed forms will no longer be sent. If you have recieved the flawed forms, please use them as you normally would. |
